In that effort, we offer you access to the HabitNu platform where you can: track your health progress, watch educational videos, connect your favorite fitness devices and set personal wellness goals and action plans. All programs allow you to create your personal HabitNu dashboard where you can track and watch your progress in real-time.

 
 
 

National Diabetes Prevention Program

Who? If you are over 18 and have other risk factors for type 2 diabetes, this evidence-based program run by the Centers for Disease Control and Prevention (CDC) is highly effective at preventing a diagnosis of diabetes. If you have been told that your blood sugar is higher than normal (prediabetes) or you get a 5 or above on the risk test, this lifestyle program is for you.

What will I do? Set your personal weight loss goal and an activity goal. Track your progress in “real time” on the HabitNu app. Progress through the CDC curriculum with your coach and team members. Practice small lifestyle behaviors that can lead to weight loss and can control your blood glucose levels. Celebrate your successes!

When? 26 1-hour sessions over 1 year, meet weekly 1st 6 months and then monthly for the 2nd 6 months


Wellness Lifestyle Coaching

Who? For anyone wishing to promote health and reach a healthy weight. Research shows that losing even 5% of your starting weight can: reduce your risk of diabetes, heart disease, stroke & some cancers, lower blood pressure, improve sleep quality, reverse insulin resistance, feel better, look better, and build self-confidence.

What will I do? Identify what is important to you. Establish baseline measures, set personal goals, and develop your action plans to practice new routines, track/assess progress, strengthen your body & cardiovascular system and manage stress. Stay accountable.
