When you join the National Diabetes Prevention Program, our Certified Lifestyle Coach will teach you what changes matter most in prevention and how to make them stick. Everyone has different challenges, and this program can help them all while making you feel great!

National Diabetes Prevention Program
Who? If you are over 18 and have other risk factors for type 2 diabetes, this evidence-based program run by the Centers for Disease Control and Prevention (CDC) is highly effective at preventing a diagnosis of diabetes. If you have been told that your blood sugar is higher than normal (prediabetes) or you have other risk factors, this lifestyle program is for you.

What will I do? Set your personal weight loss goal and an activity goal. Track your progress in “real time” on the HabitNu app. Progress through the CDC curriculum with your coach and team members. Practice small lifestyle behaviors that can lead to weight loss and can control your blood glucose levels. Celebrate your successes!
When? 26 1-hour sessions over 1 year, meet weekly 1st 6 months and then monthly for the 2nd 6 months
